Workshop 2 Autonomic Dysfunctions in patients with neurodegenerative disorders: diagnosis and management.

Workshop learning objectives:
  • To present anatomy and physiology of autonomic nervous system.
  • To present the spectrum of autonomic dysfunctions in patients with neurodegenerative/ demyelinating disorders.
  • To introduce strategies for diagnosis and effective management of autonomic dysfunctions in patients with neurodegenerative/demyelinating disorders.

This course will be of interest to physicians, medical residents, nurses, as well as physical and occupational therapists involved in care of patients with neurodegenerative/demyelinating disorders. Unstable blood pressure control including orthostatic and postprandial hypotension as well as autonomic dysreflexia, bowel and bladder disturbances, and sexual dysfunction are frequent, disabling features in patients with the following disorders: Parkinson's disease (PD), dementia with Lewy bodies, multiple system atrophy (MSA), spinal cord injury (SCI) and multiple sclerosis (MS).

In degenerative disorders, the autonomic nervous system could be entirely or selectively (either the sympathetic or the parasympathetic component) affected. Sympathetic autonomic failure is clinically characterized by cardiovascular autonomic dysfunctions, sexual dysfunction, and neurogenic bladder and bowel. Syncope associated to orthostatic hypotension, in combination of urinary bladder and bowel dissections are common symptoms in these individuals.

There are numerous non-invasive tests including standard cardiovascular tests (e.g. tilt up challenge test), urodynamic tests, gastrointestinal motility studies, sweating reflexes and pupillary responses that assess autonomic dysfunction in these patients. The study of autonomic symptoms and abnormal tests in this population are crucial in order to prevent morbidity due falls, severe constipation and to avoid side effects of drugs that interfere with autonomic function.

At the end of this course, participants will be able to explain the complexity of autonomic dysfunctions including specifics of anatomy and physiology as well as the latest evidences from basic and clinical research in diagnosis and management of autonomic dysfunctions in patients with neurodegenerative/demyelinating disorders. Participants will also have an understanding of the latest technology used in the evaluation of autonomic dysfunctions
